Notice:

The minimum age of a disaster relief volunteer is 18 years.

Notice:

Volunteers are responsible for their own transportation to and from the disaster site.
Length of stay should be no longer than 1 week for participants.
Food and housing will be provided. Housing is dormitory style. Bring your own sleeping gear (cot, sleeping bag, etc.)
Submission of this form does not guarantee placement on a team, but will help us to coordinate with Blue Hats/Team Leaders who may be serving during your available dates.
